Introduction to the Relationship Between Thyroid Problems and Male Infertility

 

The thyroid gland is a gland located in the mid-neck region in both men and women. It regulates its hormones to control the body’s metabolism and energy utilization. Two main types of problems can occur with thyroid gland function.

Hypothyroidism (Underactivity)
In hypothyroidism, the thyroid gland is less active. Men with hypothyroidism often experience symptoms of decreased metabolism, including low energy, weight gain, and there is a significant overlap in possible symptoms of low testosterone and hypothyroidism. Hypothyroidism is usually treated with an alternative thyroid hormone, and TSH levels are typically elevated in men with hypothyroidism.

Hyperthyroidism (Overactivity)
In hyperthyroidism, the thyroid gland is functionally higher than normal. Symptoms often include weight loss, sweating, and palpitations. If hyperthyroidism is suspected, the underlying cause of elevated thyroid hormone levels should be investigated. Possible causes include autoimmune thyroid inflammation (Hashimoto’s), thyroid tumors, and treatment usually involves surgical removal of the thyroid gland. TSH levels are usually low in men with hyperthyroidism.

Both underactivity and overactivity of the thyroid gland can harm the natural production of sperm. However, thyroid problems are a rare cause of male infertility. Thyroid function should be investigated in male fertility patients only if there are potential symptoms, such as a significant recent weight loss or gain, abnormal energy levels, sweating, or palpitations. Treatment of thyroid dysfunction can improve sperm parameters and fertility in men with thyroid dysfunction, while mild thyroid abnormalities generally do not lead to fertility impairments.

Evaluation of Thyroid Disorders
The initial screening test for men with symptoms suggestive of thyroid dysfunction is the TSH (Thyroid Stimulating Hormone) test. TSH is an anterior pituitary hormone that stimulates the thyroid gland to produce thyroid hormone (T4), which is then converted to the active form (T3). The normal range for TSH is 0.4-3.0 µIU/mL.

Treatment of Thyroid Problems in Men with Male Infertility
If the TSH level is abnormal, a complete evaluation by an endocrinologist or male fertility specialist is required to rule out tumors or other medical problems. At that time, T3/T4 hormone levels should also be studied.
